General Requirements: 
C.N.A. and current on the Maine State Registry Minimum of 1 year of experience as a CNA or DSP Minimum of 1 year possessing a CNA certificate
Education Requirements: 
High School Diploma or GED

Responsibilities

I.     General

  1. Provides appropriate role modeling for residents at times.
  2. Eats prepared meals with residents, modeling appropriate behavior, assuring residents safety and becoming familiar with habilitation plans.
  3. Is aware of the concept of normalization and encourages appropriate residents behavior.
  4. Respects resident’s dignity, rights and confidentiality.
  5. Is familiar with policy and procedure manual.
  6. Attends staff meetings.
  7. Attends workshops as required.
  8. Follows supervisors instructions in crisis.
  9. Advocates for residents needs.
  10. Follows established health and safety procedures.
  11. Reports any safety concerns to the Adminstrator.

 

II. Nursing Care

  1. Reports any changes in resident’s physical condition to nurse.
  2. Reports any changes in resident’s behavior to DTC.
  3. Monitors bowel habits.
  4. Learns and assists with personal hygiene for residents not involved with hab plans in these areas and who require assistant.

 

III. Habilitation Programming

  1. Becomes familiar with resident habilitation plans.
  2. Assists DTA, DT and DTC in with implementation of hab plans.
  3. Becomes familiar with behavioral interventions and assists if necessary.

 

IV. Activities

  1. Participates in in-house recreational activities with residents.
  2. Participates in community activities with residents.
  3. Assists with transportation of residents as needed.
  4. Becomes familiar with safety practices related to transporting residents in wheelchairs in van.

 

V. Record Keeping

  1. Becomes familiar with ADL check off and completes for each assigned resident.
  2. Becomes familiar with habilitation plan recording.
  3. Becomes familiar with nursing, activities and social service progress notes.
  4. Becomes familiar with employee/visitor/resident incident reports and completes them in a timely manner as needed.
  5. Record an mileage and destination.
  6. Becomes familiar with collection of baseline data.
  7. Ensures resident purchase receipts are turned over the Activities Coordinator and that any change is accounted for and returned to resident’s pouch.
  8. Ensures that receipts for petty cash expenditures are turned in to Administrator.
  9. Completes a timecard for each shift worked, accurately and in a timely manner.
  10. Records all long distance phone calls in the long distance log.

 

IV.   Environmental Management

  1. Cleans wheelchairs, braces, splints and reports damage to nurse or DON.
  2. Makes environment as safe as possible by addressing safety hazards as they arise.
  3. Keeps resident’s belongings and household items picked up.
  4. Does laundry as needed.
  5. Mends resident’s clothing.
  6. Makes beds for those residents not independent in this area of involved in a program.
  7. Assures that correct meal is obtained from the kitchen and serviced to the appropriate resident.
