Community Partners provides assistance to help assure the people we support have access to appropriate health care, housing, transportation, community relationships, employment supports and so much more. Your Annual Fund donation will help fund:
- Daily personal staff supports
- Employment Supports
- Day programs for seniors with disabilities
- Participation in self advocacy groups
- Recreational and enrichment activities
- Specialized Nursing Care
Community Partners also operates 40 residential sites that offer a variety of independent or assisting living options. Support may include wellness and personal care support; help paying bills, grocery shopping; providing transportation to doctor or other appointments. Every individual is supported in identifying and realizing their personal goals and dreams. We hold that purpose at the heart of all we do.
