and youth with developmental disabilities. Each home is unique and supports
a lifestyle that reflects the needs and interests of the children and

Transition Services: The transition
from school to work or from children’s services to adult services
can often be a confusing, poorly marked path. Community Partners participates
in the delivery of services identified by the PET process and provides
helpful supports and guidance to the child and family in the transition
to the adult services system. We draw on our vast experience in providing
progressive adult services to facilitate the transition process and
provide consultation to the family, the consumer and the special education
and children’s services systems
