Residential Supports

Community Partners offers a wide variety of services and supports to enable a person with a disability to live successfully, happily and safely in the community. We are able to provide support for people in their own homes or apartments and provide 24 hour services in more than 35 homes in Southern and Central Maine. Community Partners works closely with consumers and their families to explore each person’s needs, identify the type and level of support that is desirable and create living environments that support them on the path to their dreams. The goals of residential supports are to maximize community inclusion, increase independence, and achieve greater levels of self-determination and feelings of self-worth. Nursing & Healthcare

ICF/MR Nursing: Community Partners operates an extraordinary 12-bed ICF/MR Nursing home in Lewiston that meets the health care and personal support needs of 12 people with developmental disabilities. This is an important option for people whose physical impairments have become more complex and require 24-hour nursing care. A positive supports approach is integrated with professional nursing and health care services to promote increased independence and life skills in an environment that meets the complicated and evolving health care needs of the consumers. Professional Nursing Consultation: The consumers in residential and day programs at Community Partners are supported by professional nursing consultation. Community Employment Supports

Community Partners offers a wide range of employment and vocational options for persons with developmental disabilities. Our employment services are fully accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). We offer a combination of situational assessment, planning, job development and on-site support options in the community. We work closely with more than 30 employers in Southern Maine to offer volunteer, job shadowing and paid employment opportunities. People are prepared to enter the workforce in an active, productive role. Personal & Social Supports

Community Partners provides a variety of services to support consumers in their personal and social growth and development. These individually-tailored services rely on a variety of approaches including volunteerism, community-based events, skill-building classes and active learning, and diverse recreational pursuits. The support may be either group-based or provided individually through the consumer’s home. The goal of the program is to create a positive environment of inclusion and participation so each person can achieve personally defined goals. Supports for Seniors

The Senior Enrichment Center in Portland and the Senior Program in Biddeford are unique services providing an array of day support options to older persons with developmental disabilities. Leisure, social, recreational, arts and self-expression, life skills, health and volunteer activities are the focus of this program. All activities are designed for people with developmental disabilities who are in their retirement years. The physical environment is sunny, open, welcoming and safe for program participants. Each person has an individualized plan of service that not only meets that person’s needs, but also contributes to the social atmosphere created by a welcoming peer group. The Senior Programs offer daily activities that are center and community based, all provided in a manner that is consistent with Community Partners’ positive supports approach.
